REVISED DOCUMENTATION FROM RSA HOLDERS FOR DATA RECAPTURE (ACTIVE CONTRIBUTORS)

Letter of Employment or Staff Identification Card.

Where a Letter of Employment is provided instead of a Staff Identity Card, kindly provide any of the following means of identification.
- National Driver’s License;
- Permanent Voter’s Card;
- International Passport (mandatory for non-Nigerians).

Enrolment Slip issued by the National Identity Management Commission (NIMC)/NIN Card with NIN.

Birth Certificate or Sworn Affidavit of Age Declaration

REVISED DOCUMENTATION FROM RSA HOLDERS FOR DATA RECAPTURE (RETIREES)

Retirement Letter issued by his/her employers.

The Retiree must also provide any of the following means of identification:
- National Driver’s License
- Permanent Voter’s Card
- International Passport (mandatory for non-Nigerians)
- Enrolment Slip issued by the National Identity Management Commission (NIMC)/NIN Card with NIN.

CHANGE OF NAMES

RSA holders that have Surname or First Name change must provide below documents.

Marriage Certificate (only applicable in the case of marriage)
Newspaper publication for change of name
Sworn court affidavit
Confirmation letter for change of name from the employer for active contributors

Prior to joining PAL Pensions, he has worked in various capacities with Premium Pension Limited, Mega Lifesciences and Codix Pharma Limited.
